List of Phd forms

Every form/application to be submitted is to be duly filled in and signed by the student and is to be forwarded by concerned Faculty Adviser/ Supervisor/Co-Supervisor/RPC/Convener, DPGC (Head, of the Department).
These forms should be submitted in the Department office only. The Department office will make necessary arrangements to send the duly filled and signed form to Academic office.

All forms are also accessible on Post Student Ldap login.

  • This is to be submitted only once.
  • Every student who is admitted to the Ph.D. Programme has to submit this form, stating the date of joining.
  • This form is to be submitted in the concerned Department/Centre/IDPs.

Declaration to be submitted by Postgraduate Students who are eligible for Institute TA/RA

  • This is to be submitted only once by institute teaching assistantship students.
  • Every student who is admitted to the Ph.D. Programme eligible for institute teaching assistantship has to submit this form.
  • This form is to be submitted in the concerned Department/Centre/IDPs.

Course Registration Form for External Ph.D. Students

  • This Form is to be submitted every semester (if not registered online).
  • This form is for External category students who join back their Parent Organisation after completion of Coursework.
  • Every student has to register Online/Offline for each semester till submission of Synopsis & Thesis.
  • If External category students are not on the campus during the registration dates then these students should download this form and send the duly filled form for further processing.

Ph.D. Annual Progress Seminar (APS) Report Format- Cum - Extension of Ph.D. Registration Form

  • This report has to be submitted Annually (in September or February as the case may be) till presentation of Pre-Synopsis Seminar.
  • Every Ph.D. Student has to give APS till presentation of Pre-synopsis.
  • This is an evaluation Report of RPC on the work presented by the student after completion of each academic year.
  • The students who joined in Autumn Semester (July), after completion of 1 year, have to present their APS latest by 30th September, of every year, till submission /presentation of Pre-synopsis. The students who joined in Spring Semester (December/January), after completion of one year, have to present their APS latest by 28th/29th February, every year, till submission /presentation of Pre-synopsis.
  • The online APS report should reach the Convener, PGAPEC within 4 days from the date of presentation of APS.
  • Fine will be levied for late presentation of APS.

Ph.D. Pre-Synopsis Seminar Examination Report Format

  • This is to be submitted only once.
    (However, in exceptional cases, where the pre-synopsis presentation is unsatisfactory OR on the recommendations of RPC/PGAPEC this may be submitted again).
  • After successful presentation of Pre-Synopsis, student must submit the thesis within four months from the date of Pre-Synopsis.

Forms for Submission of Ph.D. Synopsis and Thesis for evaluation

  • This is to be submitted only once.
  • This form alongwith the copy of Qualifying Degree Certificate is to be submitted at the time of submission of Synopsis & Thesis for evaluation to the Department Office/HOD.
  • The form also includes Panel of Examiners form.
  • The PDF file of the Synopsis and Thesis to be submitted to the Department office/HOD.
  • The student must also submit an attested along with this form.

Form for Final Submission of Ph.D. Thesis (After Defence/Viva-voce Examination)

  • This is to be submitted only once.
  • This form is to be submitted after defence of viva-voce examination.
  • The student is required to complete the required formalities / clear dues, if any to become eligible to be included in the Convocation list.

Request for Verification Certificate for Obtaining LAPTOP Loan from Bank

  • This form is for verification of student details by academic section to be submitted by the student to the bank to apply for Laptop Loan.
  • This form is to be used for Laptop Loan only.

Request for Change of Category

  • The student may be required to change his/her existing admission category to a new category for various reasons. The request should be made to PGAPEC in this format.
  • Students must attach appropriate documents (For eg. In the case of change of category to External (EX), student is required to submit External Supervisor's Biodata, External Supervisor's consent, and NOC from the Employer; For Change in category to TA/CSIR/UGC, a copy of GATE/CSIR/UGC score card/award letter etc.) for consideration of PGAPEC for approval.

  • This format is to be used as and when required.
  • The consent of Existing (old) Supervisor(s) and Proposed (New) Supervisor(s) is necessary.
  • The format can be used for inclusion of External Supervisor for the Ph.D. work. This student is required to submit External Supervisor's Biodata, External Supervisor's consent, and NOC from the Employer.

Request for Extension of Annual Progress Seminar (APS)

  • This format should be submitted 15 days before the due date of APS presentation.
  • This is an application /request for seeking Extension for presentation of APS after the due date.
  • If the request has to be made on the Bonafide reasons/Medical grounds, the relevant documents/certificate are to be attached as a proof along with this form.
  • Students must note that if the APS is presented after the due date, a fine of Rs. 750/- per week is applicable.

Request for Partial Financial Support from IRCC for Completion of Synopsis & Thesis (After successful presentation of Pre-synopsis Seminar

  • This is to be submitted only once.
  • This Form is to be submitted after successful presentation of pre-synopsis seminar and completion of tenure of TAship/RAship/Fellowship.
  • The Financial support is given from the date of presentation of pre-synopsis seminar, for a maximum of 3 months or till the date of submission of synopsis and thesis, whichever is earlier, on the basis of attendance certified by the respective academic unit.

  • This is an application /request format for submission of Synopsis & Thesis from outside the Institute.
  • The student who has joined Organization/Industry for a further job before submission of Synopsis & Thesis (but after successful presentation of Pre-Synopsis Seminar is eligible for this consideration).
  • The student should seek prior approval of the competent authority before joining any University/Organization/Industry.
